Waiver and Release of Liability for 2024 CNE Zero to Five K

In consideration for being permitted to participate in the CNE ZERO TO FIVE K ("Event"), I waive to the fullest extent permitted by law for myself, my heirs and personal representatives any and all claims that I may have for damages against Childcare Network of Evanston ("CNE") and its officers, directors, agents, employees, sponsors, and volunteers, and all governmental and public entities where the Event takes place (collectively, the "Released Parties"), from any loss, liability or claims arising out of my participation in the Event, including claims for damages for death, illness including SARS-CoV-2 and related illnesses, personal injury or property damage suffered by me or any other persons, or members of my family, including any minors, who may accompany me, in connection with the Event, including pre- and post-Event activities.

I also agree to defend, indemnify and hold harmless the Released Parties, and any successors and assigns of the Released Parties to the fullest extent permitted by law, from all claims for damages by any person or entity where the claim relates to an injury to me, or to an injury to another caused in whole or in part by me.  I have been warned that I must be in good health to participate in the Event and I attest and verify that I am physically fit and have adequate insurance to cover any injury or damage I may cause or suffer while participating in the Event, or else I agree to bear the costs of such injury or damage myself.  I shall at all times comply with instructions given by Event officials.  This Waiver shall in no way obligate any Released Party (and any successors and assigns of any Released Party) to ensure my safety.

I grant permission to CNE to use my name and photographs, videos, recordings, and any other record of my participation in the Event for any purpose.

By checking the box below, I hereby attest that I am over the age of 18 and/or my legal guardian has also read and agreed to this Release.
