Department of Human Services Child Care Assistance Program (CCAP) 

The Child Care Assistance Program (CCAP) provides low-income, working families with access to high-quality, affordable child care, which allows the parents to continue working and contributes to the consistency and success of each child’s early education experience. Families are required to cost-share on a sliding scale based on family size, income and number of children in care, and must provide income documentation and proof of employment or school enrollment. CNE partners with the McGaw YMCA, Reba Early Learning Center, Waraich Family Child Care, and Swift Childcare in Skokie to provide CCAP services.

Just below, please find the CCAP applications for childcare services, in both English and Spanish.  If you are new to applying for CCAP, you may print this form to complete it and speak with your provider about where to submit it.  If you are a current CCAP client and need to complete a redetermination, please use the redetermination application mailed to you by CNE.  If you cannot locate that document, you may use the blank CCAP Redetermination form posted below.
